Mandibular Advancement Devices for Sleep Apnoea

By Loran Simon

What to consider in choosing a mandibular advancement device for sleep apnoea is a important matter. These sorts of oral appliances are proven in various research studies to effectively treat sleep apnoea. They reduce the amount of AHI's to satisfactory levels. AHI means Apnoea-Hypopnea Index, this is the number of pauses of breathing divided by the number of hours of sleep.

There are a great many mandibular advancement devices to choose from. There are also a number of different names by which they're called, such as mandibular advancement appliance, mandibular advancement splint, and mandibular repositioning device.

Despite the varied names they are all basically the same and work in the same way. Obstructive sleep apnoea is a 'mechanical problem ', the upper airway physically closes or rather collapses. This is the cause for the pause in breathing. Fortunately our bodies emergency system will get us breathing again after a short while, normally with a loud gasp or choking sound. A mandibular advancement device stops the upper airway collapsing by holding the lower jaw forward, this brings the tongue and the hyoid bone forward and so keeps the airway open while asleep.

Almost all mandibular advancement devices are made of plastic. Plastic is a low cost material, that's easy to work with and adjust. Plastic devices are inherently bulky due to the strength they need to have. This bulkiness can make them difficult for people to endure, the mouth is extremely sensitive and the gag reflex is easily triggered by large foreign objects in the mouth. The bulkiness also has a tendency to open the mouth vertically, this reduces the airway and means the lower jaw must be brought further forward which can often be uncomfortable. The vertical opening also means the lips can't be closed, causing the wearer to breath through the mouth which can lead to a dry mouth and sore throat. The plastic materials all deform and degrade in the mouth whilst becoming infested by oral fungi which makes them manky and pungent. After a relatively short time every plastic device needs to be replaced.

The Somnowell mandibular advancement appliance is the first device to be made of chrome cobalt alloy. Like gold it is ideal for long term use in the mouth. Chrome cobalt is extremely strong in thin section so the device is very small and discreet. The material is impervious to oral fungi and can even be boiled. As well as being exceedingly comfortable the Somnowell device will looks and feel exactly the same after many years of use. Most importantly the Somnowell is a good treatment for sleep apnoea.
